Disability Accommodation Process

The Americans with Disabilities Act ("ADA") defines the process to engage in dialogue with employees. The purpose of the ADA Process is to engage in an interactive dialogue for the purposes of enabling an employee to perform their usual and customary duties with or without accommodation. This dialogue allows the employee and the employer to engage in a conversation that will allow you to return/ or remain working. 

If you wish to engage in this ADA process, complete review the ADA Intake Application and complete the forms to begin; any information gathered as part of the ADA process is confidential.
