NOTE: The Bullying and Intimidation Complaint Form may, on occasion, be used to report sex-based harassment, in which case it becomes a Title IX issue. All sex-based harassment complaints are to be handled following the protocol outlined in AR 0410. The Sexual Harassment Report Check-List and/or Sexual Harassment Flowchart will help with step-by-step instructions. The School Investigation Report Summary [E(2) 5131.2] is to be submitted to the Quality Assurance Office per the instructions at the bottom of the form.
